Keep your eyes on the patient. The SOAP note is written for you.

Verbato writes a SOAP consultation note from the recording of your consultation. A SOAP note records a consultation in four parts: Subjective, what the patient reports; Objective, what is examined and measured; Assessment, the clinician's reading; and Plan, what happens next.

For doctors who write a note after every consultation.

How your SOAP consultation note is written

  • What the patient reported is kept apart from what you observed or concluded.

  • Your assessment keeps the certainty you gave it: “probable” stays “probable”.

  • Nothing is added that was not said: no symptom, value, diagnosis or plan.

  • Abbreviations that are easy to misread are written out in full.

Your format, not ours

The sections of your SOAP consultation note and the way it is written are yours to change. To fix a document, say what is wrong in your own words.

  1. 1You correct it

“The Subjective runs too long. Keep it short.”

That SOAP consultation note changes. Nothing else does.

  2. 2You say it again

Make the same correction on a later SOAP consultation note and Verbato asks: “You've changed this before. Always?”

  3. 3You decide

Answer “Always” and it is kept as a rule, applied from your next SOAP consultation note on. Answer “Just this once” and nothing is kept. Documents already written stay as they are, and nothing becomes a rule unless you confirm it.

  4. 4Your rules

    I keep “Subjective” short.Undo

Every rule you have kept is listed, and you can undo any of them. An undone rule stops applying from your next SOAP consultation note.

How it works

Upload a file, paste a link, record in your browser, or send a voice note by Telegram or email. Verbato makes a word-for-word transcript, which you can edit and which stays beside the document. Your SOAP consultation note is written from it, in minutes.

Recording limits

A recording made in your browser can run up to 30 min on every plan; a longer session goes in as a file you upload. A free account takes recordings of up to 40 min each; for longer ones, see pricing.

Your recording

Deleted after 7 days by default, together with its transcript and documents. You can keep a project longer, up to your plan's limit. Deleting a recording leaves a receipt you can download. Nothing you record is used to train or improve AI models.

Questions about the SOAP consultation note

Do I have to say exam findings and vital signs out loud?
Yes. The note is written from the recording, so it holds only what was said. If no exam findings or results were stated, the Objective section says so in one line instead of filling any in.
Does one recording give me both the note and the patient's summary?
Yes. A recorded consultation comes back as both: this note for the record, and a plain-language summary for the patient to take home. Each is written from the same transcript.
What if I name a medicine but leave out the dose?
The Plan lists the medicine as you said it. A dose, route or frequency you did not state is not filled in.
Does it add diagnosis codes or billing codes?
Only if you say them. The note holds what was said in the consultation, and nothing is added to it, codes included.

Record your next consultation. The note is yours to review.
