What you told the patient, written so the patient can follow it.

Verbato writes a patient visit summary from the recording of your consultation. A patient visit summary tells the patient, in plain words, what the visit found and what to do next: the medications, the follow-up and the signs that mean they should seek help.

For clinicians who send patients home with written instructions.

What is said · transcript excerpt

  1. 09:48
    Doctor

I'm starting you on amoxicillin. It's an antibiotic, for the infection. One gram every eight hours, for seven days.

  2. 10:02
    Patient

    Every eight hours. OK.

  3. 10:05
    Doctor

And I want to see you again in forty-eight hours, to check how you're responding. If your breathing gets harder before then, don't wait, come back sooner.

  4. 10:20
    Patient

    Got it. Thank you, doctor.

How your patient visit summary is written

  • Plain words and short sentences, addressed to the patient as “you”.

  • A medical term that cannot be avoided is explained once, in parentheses.

  • No alarm, and no reassurance beyond what you said in the room.

  • Instructions come as short steps, the most important one first.

Your format, not ours

The sections of your patient visit summary and the way it is written are yours to change. To fix a document, say what is wrong in your own words.

  1. 1You correct it

“Number the steps instead of using bullets.”

That patient visit summary changes. Nothing else does.

  2. 2You say it again

Make the same correction on a later patient visit summary and Verbato asks: “You've changed this before. Always?”

  3. 3You decide

Answer “Always” and it is kept as a rule, applied from your next patient visit summary on. Answer “Just this once” and nothing is kept. Documents already written stay as they are, and nothing becomes a rule unless you confirm it.

  4. 4Your rules

    I number lists.Undo

Every rule you have kept is listed, and you can undo any of them. An undone rule stops applying from your next patient visit summary.

How it works

Upload a file, paste a link, record in your browser, or send a voice note by Telegram or email. Verbato makes a word-for-word transcript, which you can edit and which stays beside the document. Your patient visit summary is written from it, in minutes.

Recording limits

A recording made in your browser can run up to 30 min on every plan; a longer session goes in as a file you upload. A free account takes recordings of up to 40 min each; for longer ones, see pricing.

Your recording

Deleted after 7 days by default, together with its transcript and documents. You can keep a project longer, up to your plan's limit. Deleting a recording leaves a receipt you can download. Nothing you record is used to train or improve AI models.

Questions about the patient visit summary

Does it add standard warning signs or advice for the diagnosis?
No. When to seek help lists only the warning signs you mentioned during the visit, and what you said to do about them. If you want the patient to have them in writing, say them aloud.
What if I never say when the patient should come back?
The summary comes back without a Follow-up section. Verbato does not guess a date or an interval. The same goes for Medications when none were discussed.
How does it write the medicines I prescribed?
Each medicine you mentioned gets a full sentence in everyday words, like the one in the example. A detail you did not say is not filled in.
Does the summary go straight to the patient?
No. It comes to you. Read it, correct anything that is off, then print it or send it through the channel you already use with patients. It exports as a PDF or a Word document.
Does it need a recording of its own, separate from the clinical note?
No. Both are written from the same recorded consultation: the note in clinical language for the record, the summary in plain language for the patient.

Say it once in the room. Review it, then hand it over.
