verbato
Therapy notes · SOAP / DAP

Your notes are drafted before your next client sits down.

Record the session or dictate a two-minute recap — either way it comes back as a draft note you review before you file: a SOAP or DAP session note, an intake assessment, a treatment plan. Quotes stay verbatim only where they matter, and nothing is invented.

Your format, not ours

Progress notes in your practice's format.

Start from SOAP or DAP, an intake assessment, or a treatment plan — or describe your own note to the assistant in about five minutes. Dictated recaps are the common path here and pair naturally with the record button, so a note is written before your next client sits down.

Your data, on a timer

Everything you send is deleted after 7 days by default; keep a project longer when you need to, one at a time, and every deletion writes you a receipt. Your recordings are never used to train or improve AI models, and never sold. You stay in control of your data — short retention by default, deletion you can prove. And the note comes back drafted, not final — you read it, edit if needed, and file it yourself.

Questions

The short answers.

Can it write SOAP or DAP notes from a recorded session?

Yes — from the session itself or a short spoken recap afterward, Verbato returns a SOAP or DAP note in the shape you set. You choose the format once and every session after that follows it. It comes back drafted, for you to review and file.

Can I use my practice's own note format?

Yes. Set up your note template and instructions once, or describe your format to the assistant in about five minutes, and every recording comes back in it.

Does it diagnose or assess risk?

No. Verbato writes the note from what was said — it never invents a diagnosis, and it never computes a risk level. Where risk is mentioned it's kept as a verbatim quote for you to act on, not a judgment the software makes.

What happens to the recording?

By default it's deleted after 7 days, every deletion writes you a receipt, and nothing you record is used to train AI models. Retention is yours to control.
