One consult in. The SOAP record out, problems numbered.

Verbato writes a veterinary SOAP note from your exam-room recording. A veterinary SOAP note is the medical record of a consult: the patient's signalment, the owner's history (Subjective), the exam (Objective), the problems found (Assessment) and the plan for each.

For small-animal vets who write up every consult.

How your veterinary SOAP note is written

  • Signalment leads the record: species, breed, age, sex, weight.

  • Problems are numbered, and the Plan answers each one by number.

  • History is attributed to the owner, findings to you.

  • Client communication and follow-up go in the Plan when you mention them.

Your format, not ours

The sections of your veterinary SOAP note and the way it is written are yours to change. To fix a document, say what is wrong in your own words.

  1. 1You correct it

    “Add a Client communication section.”

That veterinary SOAP note changes. Nothing else does.

  2. 2You say it again

Make the same correction on a later veterinary SOAP note and Verbato asks: “You've changed this before. Always?”

  3. 3You decide

Answer “Always” and it is kept as a rule, applied from your next veterinary SOAP note on. Answer “Just this once” and nothing is kept. Documents already written stay as they are, and nothing becomes a rule unless you confirm it.

  4. 4Your rules

    I include a “Client communication” section.Undo

Every rule you have kept is listed, and you can undo any of them. An undone rule stops applying from your next veterinary SOAP note.

How it works

Upload a file, paste a link, record in your browser, or send a voice note by Telegram or email. Verbato makes a word-for-word transcript, which you can edit and which stays beside the document. Your veterinary SOAP note is written from it, in minutes.

Recording limits

A recording made in your browser can run up to 30 min on every plan; a longer session goes in as a file you upload. A free account takes recordings of up to 40 min each; for longer ones, see pricing.

Your recording

Deleted after 7 days by default, together with its transcript and documents. You can keep a project longer, up to your plan's limit. Deleting a recording leaves a receipt you can download. Nothing you record is used to train or improve AI models.

Questions about the veterinary SOAP note

How do exam findings get into the record if I examine in silence?
They don't. The record is written from what is said in the room, so talk through the exam as you go: vitals, findings by system, results.
Does one recording give me the record and the owner's summary?
Yes. A recorded consult comes back as both: this SOAP note for the record, and a plain-language summary for the owner to take home. Each is written from the same transcript.
What if I don't state the weight or the neuter status?
That field is left out of the signalment. Nothing is guessed from the rest of the conversation, and no dose is worked out from a weight you did not give.
Is it only for dogs and cats?
It was written for small-animal general practice, and the record is not tied to a species. The signalment takes the species and breed you state, and the sections stay the same.
Will it suggest differentials or doses?
No. The Assessment lists the differentials you stated for each problem, and the Plan carries the doses you said. Verbato adds no differential, no dose and no test of its own.
