Say what changed since the last visit. Get the recheck note.

Verbato writes a recheck note from your exam-room recording. A recheck note records a follow-up visit for a problem already under treatment: what changed since the last visit, a focused exam, how the patient responded and how the plan changes.

For vets who follow a known problem from visit to visit.

    So the drops are working?

It's improved. Continue the ear drops for five more days, then stop. I only need a recheck if the signs come back.

How your recheck note is written

  • The note opens with the problem being rechecked, and stays on it.

  • The interval history is the owner's account, attributed to the owner.

  • Missed doses and how the treatment was given are recorded as the owner reported them.

  • No finding, dose or plan is added that was not said in the room.

Your format, not ours

The sections of your recheck note and the way it is written are yours to change. To fix a document, say what is wrong in your own words.

  1. 1You correct it

    “Keep the Interval history short.”

That recheck note changes. Nothing else does.

  2. 2You say it again

Make the same correction on a later recheck note and Verbato asks: “You've changed this before. Always?”

  3. 3You decide

Answer “Always” and it is kept as a rule, applied from your next recheck note on. Answer “Just this once” and nothing is kept. Documents already written stay as they are, and nothing becomes a rule unless you confirm it.

  4. 4Your rules

    I keep “Interval history” short.Undo

Every rule you have kept is listed, and you can undo any of them. An undone rule stops applying from your next recheck note.

How it works

Upload a file, paste a link, record in your browser, or send a voice note by Telegram or email. Verbato makes a word-for-word transcript, which you can edit and which stays beside the document. Your recheck note is written from it, in minutes.

Recording limits

A recording made in your browser can run up to 30 min on every plan; a longer session goes in as a file you upload. A free account takes recordings of up to 40 min each; for longer ones, see pricing.

Your recording

Deleted after 7 days by default, together with its transcript and documents. You can keep a project longer, up to your plan's limit. Deleting a recording leaves a receipt you can download. Nothing you record is used to train or improve AI models.

Questions about the recheck note

Does it compare this visit with my previous note?
Only through what you say. Verbato does not read your earlier records, so state the comparison aloud: “marked at the last visit, mild now”. The note carries it as you said it.
What if the plan does not change?
The Plan adjustment section says so in one line. It does not restate the earlier plan or add a follow-up you did not mention.
Do I have to use set words for the response to treatment?
No. Your read is recorded in your own words, whatever they are. If you do not state one, the note may sum up the change your findings show; check that line before you sign.
How is a recheck note different from a full SOAP note?
It is narrower. A recheck note follows one known problem: the interval history, a focused exam, your read on the response and any change to the plan. A new problem belongs in a full SOAP note.

Record the recheck. Read the note. Close the visit.
