Your discharge instructions, in words the owner can follow at home.

Verbato writes a client take-home summary from your exam-room recording. A client take-home summary tells the owner, in plain words, what the visit found, what was done, how to care for the animal at home and the signs to call about.

For vets who send owners home with written instructions.

For the next three days, a bland diet, in small meals. And keep fresh water available for him at all times.

He's not going to like that.

He'll cope. If Milo is still vomiting after twenty-four hours, call the clinic. He'll need X-rays of his belly.

What we found

Milo has an upset stomach, most likely from something he ate out of the bin. He is also a little dehydrated.

Treatments done today

We gave Milo an injection to stop the vomiting and fluids under the skin to rehydrate him.

Home care

  • Feed a bland diet in small meals for 3 days. (appears in this excerpt)
  • Keep fresh water available at all times. (appears in this excerpt)

Warning signs

If Milo is still vomiting after 24 hours, call the clinic — he will need X-rays of his belly. (appears in this excerpt)

Follow-up

Please call us tomorrow to let us know how he is doing. (appears in this excerpt)

Medications — also part of a client take-home summary; nothing in this example's recording calls for it, so it is left out.

How your client take-home summary is written

  • Plain, everyday words, and no clinical abbreviations at all.

  • Every medication direction is a full sentence, with or without food included.

  • Warm but factual: no alarm, and no reassurance beyond what you said.

  • Home care comes as short steps, in order of importance for the animal.

Your format, not ours

The sections of your client take-home summary and the way it is written are yours to change. To fix a document, say what is wrong in your own words.

  1. 1You correct it

“Call the Warning signs section ‘When to call us’.”

That client take-home summary changes. Nothing else does.

  2. 2You say it again

Make the same correction on a later client take-home summary and Verbato asks: “You've changed this before. Always?”

  3. 3You decide

Answer “Always” and it is kept as a rule, applied from your next client take-home summary on. Answer “Just this once” and nothing is kept. Documents already written stay as they are, and nothing becomes a rule unless you confirm it.

  4. 4Your rules

I call the “Warning signs” section “When to call us”.Undo

Every rule you have kept is listed, and you can undo any of them. An undone rule stops applying from your next client take-home summary.

How it works

Upload a file, paste a link, record in your browser, or send a voice note by Telegram or email. Verbato makes a word-for-word transcript, which you can edit and which stays beside the document. Your client take-home summary is written from it, in minutes.

Recording limits

A recording made in your browser can run up to 30 min on every plan; a longer session goes in as a file you upload. A free account takes recordings of up to 40 min each; for longer ones, see pricing.

Your recording

Deleted after 7 days by default, together with its transcript and documents. You can keep a project longer, up to your plan's limit. Deleting a recording leaves a receipt you can download. Nothing you record is used to train or improve AI models.

Questions about the client take-home summary

Does it add the usual warning signs for the condition?
No. Warning signs lists only what you told the owner to watch for, and what you said to do: call the clinic, go to emergency. Say them in the room if you want them on the handout.
What happens to medical terms the owner will not know?
They are replaced with everyday words. A term that cannot be avoided is explained once, in parentheses, the first time it appears.
Is the summary written from the same recording as the SOAP note?
Yes. Both come from one exam-room recording: the SOAP note in record language for the file, the summary in plain language for the owner.
Does the summary go straight to the owner?
No. It comes to you. Read it, correct anything that is off, then print it or send it the way your clinic already contacts owners. It exports as a PDF or a Word document.

Say it in the exam room. Review it, then send it home.
