Therapy notes · SOAP / DAP

Your notes are drafted before your next client sits down.

Record the session or dictate a two-minute recap — either way it comes back as a draft note you review before you file, and every session after follows your format. Quotes stay verbatim only where they matter, and nothing is invented.

DAP note

Data

Client reports sleep improved to ~6 hours; early waking ~04:00 persists. Wind-down routine used 4/7 nights.

Assessment

Partial response to sleep-hygiene plan; early-morning awakening unchanged.

Plan

Continue routine; add stimulus-control step. Review in two weeks.

The Therapy pack

What Verbato writes from a session.

Start from SOAP or DAP, an intake assessment, or a treatment plan — every one of them yours to adjust, and the dictated recap pairs naturally with the record button.

Session note — SOAP

Subjective · Objective · Assessment · Plan — drafted for your review.

Session note — DAP

Data · Assessment · Plan — the same session, your format.

Intake assessment

The first session, structured — history and presenting concerns.

The one you define

There’s no menu — describe your note once and it’s a type like any other.

The proof — a real recording, the real documents

A real recap — and the session note Verbato wrote from it.

A two-minute spoken recap is enough: the note comes back drafted, quotes verbatim only where they matter, nothing invented.

DAP note

Data

Client reports sleep improved to ~6 hours; early waking ~04:00 persists. Wind-down routine used 4/7 nights.

The mechanism

From session to filed note.

01

Record — or recap

The session itself, or a two-minute spoken recap after the client leaves.

02

It becomes the verbatim record

Word for word — quotes stay quotes, and nothing is invented.

03

Your note writes itself into your format

SOAP or DAP, in your practice’s shape — you review it, then you file it.

A correction you confirm applies from the next session on — and you can undo any of them.

Conduct, not badges

A session is not content.

Deleted fast, provably, never fed to a model to learn from. And the note comes back drafted, not final — you read it, edit if needed, and file it yourself.

  • Deleted in 7 days by default — recording, transcript and documents together.
  • Deletion leaves a receipt you can check, not take on faith.
  • Never used to train or improve AI models — ours or anyone else’s.
  • Delete everything, any time, on demand.

Hours are the only meter.

Documents, corrections and exports are never counted — you choose hours, everything else is included. There’s a free tier, no card.

The short answers

Fair questions.

Can it write SOAP or DAP notes from a recorded session?

Yes — from the session itself or a short spoken recap afterward, Verbato returns a SOAP or DAP note in the shape you set. You choose the format once and every session after that follows it. It comes back drafted, for you to review and file.

Can I use my practice's own note format?

Yes. Set up your note template and instructions once, or name your note and correct the example the assistant writes, and every recording comes back in it.

Does it diagnose or assess risk?

No. Verbato writes the note from what was said — it never invents a diagnosis, and it never computes a risk level. Where risk is mentioned it's kept as a verbatim quote for you to act on, not a judgment the software makes.

What happens to the recording?

By default it's deleted after 7 days, every deletion writes you a receipt, and nothing you record is used to train AI models. Retention is yours to control.
