The risk conversation, written down as it was said. Nothing inferred.

Verbato writes a risk assessment note from the recording of your session. A risk assessment note records what a client said about risk to themselves or others, the factors that raise or lower it, and the safety plan agreed in the session.

For clinicians who document a risk or crisis conversation.

Start with a recording

  • YouTube, Spotify, Google Drive, Dropbox and more than 1,000 sites.
  • Free plan: 2 h of audio a month. Files up to 40 min.
  • Without an account, your recording is held for 24 hours. It starts when you create your account; if you don't, it is deleted.

How your risk assessment note is written

  • The client's own words about harm are quoted exactly, never softened or paraphrased.

  • What was not asked is “not assessed”. It is never written as a denial.

  • Risk and protective factors are the ones discussed, each attributed to what was said.

  • Ideation, intent, plan and means are each recorded on their own.

Your format, not ours

The sections of your risk assessment note and the way it is written are yours to change. To fix a document, say what is wrong in your own words.

  1. 1You correct it

“Write dates in full, like 4 August 2026.”

That risk assessment note changes. Nothing else does.

  2. 2You say it again

Make the same correction on a later risk assessment note and Verbato asks: “You've changed this before. Always?”

  3. 3You decide

Answer “Always” and it is kept as a rule, applied from your next risk assessment note on. Answer “Just this once” and nothing is kept. Documents already written stay as they are, and nothing becomes a rule unless you confirm it.

  4. 4Your rules

I write dates with the month in words.Undo

Every rule you have kept is listed, and you can undo any of them. An undone rule stops applying from your next risk assessment note.

How it works

Upload a file, paste a link, record in your browser, or send a voice note by Telegram or email. Verbato makes a word-for-word transcript, which you can edit and which stays beside the document. Your risk assessment note is written from it, in minutes.

Recording limits

A recording made in your browser can run up to 30 min on every plan; a longer session goes in as a file you upload. A free account takes recordings of up to 40 min each; for longer ones, see pricing.

Your recording

Deleted after 7 days by default, together with its transcript and documents. You can keep a project longer, up to your plan's limit. Deleting a recording leaves a receipt you can download. Nothing you record is used to train or improve AI models.

Questions about the risk assessment note

Does Verbato assess or rate the risk?
No. The note records what was said and who said it. Your own stated judgment goes in the Clinician's assessment section. If you did not state a risk level, the note says exactly that.
What if no safety plan was made in the session?
The note says so in one line. Verbato never writes a plan, a referral or a crisis resource that you did not state.
What if the client made no statement about harm?
The first section says so in one line. Factors that were not covered are marked “not assessed”.
Can I dictate the note after the session instead?
Yes. Dictate your own account once the client has left. Your statements are treated as your clinical observations and judgments, and the client's words are quoted only where you quote them.
How do I know a quote is the client's and not mine?
Each quote is attributed to its speaker, and the word-for-word transcript beside the note shows who said what. Check the quotes against it before you file the note.

What was said, in writing. The assessment stays yours.
